Initiation Summary Report
Share |

Article X of the Constitution requires that all initiation forms and fees be submitted to International Headquarters within thirty (30) days of the Initiation ceremony. Failure to comply may result in suspension of the Inn/Hall’s Charter.

Encourage compliance by having initiates pay their fees online prior to initiation. Collect payment confirmations as proof of payment and submit this form to HQ immediately after initiation.

Initiation Fees are collected in U.S. dollars and are non-refundable.

Fees are to be paid by individual initiate online, however, if college/university restrictions apply, the Inn Exchequer must collect fees and ensure that they are remitted to [H]eadquarters in a manner consistent with college/university required procedures.

Note that any local dues above the constitutionally required initiation fee, must be collected by the Inn and not sent to Headquarters.
